SUNRISE, Fla., May 6, 2014 /PRNewswire/ -- Sunshine Health, a subsidiary of Centene Corporation, has earned a prestigious national award at the Dorland Health's Fifth Annual Case in Point Platinum Awards in Washington, D.C. Sunshine Health won first place for its successful Long-Term Care (LTC) plan and received an honorable mention for its Transitions of Care program. The Case in Point Platinum Awards program recognizes case management programs that successfully deliver sustained efficiency and effectiveness across a variety of disciplines and settings in the continuum of care coordination.

In August 2013, the Agency for Healthcare Administration (AHCA), Sunshine Health, and other health plans launched the first phase of the Statewide Medicaid Managed Care (SMMC) contract. Now that the contract is fully implemented, Sunshine Health offers LTC services in 10 of Florida's 11 regions. This transition of care model is the largest implementation of its kind to take place nationwide.

Sunshine Health's award winning LTC plan offers its members the option to safely stay in their homes or in an Assisted Living Facility (ALF) as an alternative to nursing home care.
