PLANO, Texas, May 6, 2014 /PRNewswire/ -- Reliant Post-Acute Care Solutions, a national leader in post-acute care networks, today announced the appointment of Robert W. Harrington, MD, SFHM, as Chief Medical Officer.

Dr. Harrington will provide clinical leadership and oversight of the evidence-based clinical protocols central to Reliant's Post-Acute Care Network (PACN) model of care. Board certified in family medicine, Dr. Harrington is a member of the Board of Directors and President-elect of the Society of Hospital Medicine. While he most recently served as Chief Medical Officer of Locum Leaders, the majority of Dr. Harrington's career has been in executive leadership positions in hospitalist programs dedicated to patient care across the post-acute continuum.

According to MedPAC, one out of every five Medicare beneficiaries is readmitted to the hospital within 30 days of discharge, costing Medicare over $17 billion dollars annually. Research shows that up to 90 percent of those readmissions are unplanned and likely the result of a break in the clinical process or poor care coordination.
